Panalist 3 Dr Samrawit Tasew. Assistant professor on Anesthesiology and Critical care, Operation theater director
The panel disusison emphasied on the need to shift the focus from the "sharp end" of care delivery to the "blunt end" of hospital leadership. Menaing that patient safety is not just about the actions of frontline providers, but rather the systems and processes put in place by hospital administrators and policymakers to prevent medical errors from occurring in the first place. "Patient safety should not depend solely on the vigilance of healthcare workers," said Dr Desalegn. "We must make it difficult for errors to happen through robust safety mechanisms and a strong culture of prioritizing patient wellbeing."
The panel highlighted the Ministry of Health's new five-strategy initiative, which gives a renewed focus on patient safety nationwide.
Dr. Samrawit Director of OR also shared St. Paul’s Hospital Initiative of routine safety checkpoints for surgical patients and double-checking protocols are currently being practiced. We also need take ownership of patients saftey Initiative across the organization. She also shared that getting the entire team on board with a culture of safety took time, but we are now starting to see the result.
Recognizing the inherent complexities of human-to-human interaction in healthcare, the panel emphasized the need for even higher safety standards compared to industries like aviation, where human-to-machine interactions are more readily measured and monitored.
We must establish clear metrics and reporting systems to track patient safety metrics. The discussion also addressed the importance of anonymous incident reporting and support systems for healthcare workers affected by medical errors, a phenomenon known as the "second victim". We cannot ignore the emotional toll that patient safety incidents can take on our providers. We should benchmark Initiatives like the University of Missouri's program for physician support are crucial in fostering a culture of openness and trust.
As MOH prepares to launch its new patient safety guidelines on September 7th, the panel's message was clear: patient safety must be the top priority for healthcare organizations, with leadership driving the change and frontline staff empowered to uphold the highest standards of care. This is not just about St. Paul Hospital," concluded Dr. Abas. We are committed to working with the Ministry of Health and the World Health Organization to integrate patient safety into the very fabric of our healthcare system, from undergraduate education to nationwide policies.
