According to the report, the Secret Service never established a joint communications center with local police and instead received only five phone calls and three text messages regarding Thomas Crooks, preventing Trump's protective detail from being warned about the suspicious individual before the shooting.

Crooks flew a drone over the rally site hours before the attack, but the flight went undetected because the Secret Service's counter-drone system was inoperable. The system was being operated by a single undertrained agent who had not tested it before the event and spent hours attempting to fix it, allowing Crooks to conduct a nearly nine-minute drone flight unnoticed.

The Inspector General recommended improvements to information sharing and addressing line-of-sight vulnerabilities ahead of future events. The Secret Service said it agreed with the recommendations and stated that many have already been implemented as part of its reform efforts.
